Job summary

Thundermist Health Center in Rhode Island seeks a Family Medicine Physician to provide full-scope primary care across the lifespan. You will manage your own patient panel, deliver inpatient hospital services, and work in a team-based model including behavioral health and care management across Woonsocket, RI and nearby sites.

You’ll support care coordination, serve diverse underserved populations, and participate in HRSA/quality initiatives while maintaining compliance with relevant regulations.

Qualifications

• MD/DO with completed residency (minimum 3 years) in Family Medicine.
• Board Certification in Family Medicine within two years of start.
• Full, unrestricted Rhode Island medical license (or ability to obtain).
• Current DEA registration and Rhode Island Controlled Substance Registration.
• Current BLS; ACLS, PALS, and NRP as applicable.
• Ability to enroll with Medicare, Medicaid, and commercial payers (CAQH).

Responsibilities

• Provide comprehensive primary care for an assigned patient panel across the lifespan.
• Manage scheduled visits, acute care, chronic disease, and preventive care.
• Perform comprehensive histories, exams, and diagnostic assessments; order tests.
• PRESCRIBE medications, including controlled substances, within scope of practice.
• Coordinate referred services including specialty care, pharmacy, and ancillary services.
• Provide crisis intervention and triage, escalating as needed.
• Provide inpatient hospital services and attending responsibilities at partner hospital(s).
• Participate in after-hours call and coverage per the schedule.
• Collaborate with behavioral health clinicians and care teams; support care coordination.

About the Role

Are you a family medicine physician who wants to practice full-scope primary care in a community that truly needs you? Thundermist Health Center is looking for Family Medicine Physicians to join our team across Rhode Island. We have openings at our Woonsocket, West Warwick, and South County locations — each serving a diverse, largely medically underserved patient population. You’ll manage your own patient panel across the lifespan, provide inpatient hospital services, and work within a team-based, integrated care model that includes embedded behavioral health, care management, and community health workers. Thundermist is a multi-site FQHC serving over 65,000 patients, and we’re committed to delivering high-quality care regardless of insurance status, immigration status, or ability to pay.

What You’ll Do

Clinical Care & Panel Management

• Provide comprehensive primary care for an assigned patient panel across the lifespan — pediatrics, adolescents, adults, and geriatrics
• Manage scheduled visits, acute care, chronic disease management, preventive care, and women’s health
• Perform comprehensive health histories, physical examinations, and diagnostic assessments; order, interpret, and follow up on labs, imaging, and testing
• Prescribe medications, including controlled substances, within scope of practice
• Coordinate referred services including specialty care, pharmacy, and ancillary services
• Provide crisis intervention and triage, escalating to appropriate levels of care as indicated

Hospital Coverage & Team-Based Care

• Provide inpatient hospital services and attending responsibilities at Thundermist’s partner hospital(s)
• Participate in after-hours call and coverage per the established call schedule
• Collaborate alongside behavioral health clinicians, psychiatric providers, nurses, MAs, care managers, community health workers, and social work staff
• Support warm hand-offs, integrated behavioral health workflows, and care coordination for high-risk and complex patients
• Coordinate with community agencies and social service partners to address social determinants of health

Quality, Documentation & Compliance

• Complete clinical documentation in the EHR (eClinicalWorks) accurately and within established timeframes
• Support HRSA UDS reporting and value-based care performance through consistent care gap closure, screening completion, and preventive care delivery
• Participate in chart audits, peer review, and quality improvement activities
• Maintain practice consistent with HRSA, OSHA, HIPAA, CLIA, and applicable state and federal regulations
• Precept physician residents, medical students, or advanced practice clinicians as assigned

What We’re Looking For

You’re a family physician who chose this specialty because you want to take care of whole families, whole communities. You’re comfortable managing a newborn well-child visit, an uncontrolled diabetic, and a complex geriatric patient in the same session — and you want to do it in a setting where your work has a direct impact on health equity.

• M.D. or D.O. with completed residency (minimum 3 years) in Family Medicine
• Board Certification in Family Medicine (ABFM or ACOFP) within two years of start
• Full, unrestricted Rhode Island medical license (or ability to obtain)
• Current DEA registration and Rhode Island Controlled Substance Registration
• Current BLS certification; ACLS, PALS, and NRP as applicable
• Ability to obtain and maintain hospital privileges at Thundermist’s partner hospital(s)
• Eligibility to enroll with Medicare, Medicaid, and commercial payers (current CAQH profile required)

Preferred:

• Prior experience in an FQHC, community health center, or safety-net primary care setting
• Bilingual (Spanish/English or Portuguese/English) strongly preferred
• Experience with team-based care models, integrated behavioral health, and value-based care
• Experience precepting residents or medical students
• Eligibility for NHSC or Rhode Island state loan repayment programs

Schedule & Locations

Full-time (40+ hours/week), benefit-eligible. A minimum of 32 clinical hours on-site per week plus inpatient hospital hours and after-hours call as scheduled. Clinic hours across all Thundermist locations are Monday through Friday, 8:00 a.m. to 8:00 p.m. Coverage may span multiple sites. Part‑time arrangements may be considered.
