Job summary

Coleman Health Services in Akron, Ohio, is seeking a Manager of OhioRISE Care Coordination/Case Management to lead a Care Coordination team and drive wraparound services for youth and families in Summit County.

The role requires an LSW or LPC or higher, supervisory experience in community-based services, and at least 3 years in children's mental health; ability to obtain NPI and Ohio Medicaid numbers is supported.

Qualifications

• Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) or higher.
• Demonstrated supervisory or managerial experience in community-based services.
• Minimum of 3 years of experience in children's mental health or related public sectors, providing community-based services to children, youth, families, or caregivers.
• Experience managing complex cases involving multi-system youth.
• Monitor timely and accurate documentation and coach staff to performance.
• Maintain an outstanding driving record.
• Ability to obtain NPI and Ohio Medicaid numbers (assistance available).

Responsibilities

• Provide strategic oversight for structured service planning and care coordination, centered on high-fidelity wraparound principles.
• Develop and supervise a team of Care Coordinators.
• Actively engage and develop relationships with child-serving systems and behavioral health providers.
