Job summary

Coleman Health Services in Mansfield, OH is hiring a Manager of OhioRISE Care Coordination/Case Management to lead a Care Coordination team serving youth and families in Richland County. You will align wraparound practices, supervise care coordinators, and build partnerships with child-serving systems.

The role requires an LSW/LPC or higher licensure, at least 3 years in children's mental health, and a proven ability to coach staff, manage complex cases, and maintain compliant documentation.

Qualifications

• Requires licensure as LSW or LPC or higher.
• Minimum 3 years in children's mental health or related public sectors.
• Experience supervising community-based services and managing multi-system youth cases.
• Ability to ensure timely and accurate documentation and staff coaching.
• Must maintain an outstanding driving record and ability to obtain NPI and Ohio Medicaid numbers.

Responsibilities
