Coleman Health Services in Mansfield, OH is hiring a Manager of OhioRISE Care Coordination/Case Management to lead a Care Coordination team serving youth and families in Richland County. You will align wraparound practices, supervise care coordinators, and build partnerships with child-serving systems.

The role requires an LSW/LPC or higher licensure, at least 3 years in children's mental health, and a proven ability to coach staff, manage complex cases, and maintain compliant documentation.

Lead the Future of Ohio Youth as a Manager of OhioRISE Care Coordination/Case Management with Coleman Health Services.

Join us as the Supervisor of OhioRISE Case Management/Care Coordination where you will lead a Care Coordination team that drives positive change in the lives of youth and families within our community. We are looking for a manager to be based out of Mansfield and serve Richland County, Ohio. Your Impact as a Manager of OhioRISE Care Coordination/Case Management:

• Provide strategic oversight for structured service planning and care coordination, centered on high-fidelity wraparound principles.
• Develop and supervise a team of Care Coordinators.
• Actively engage and develop relationships with child-serving systems and behavioral health providers.
• Initiate quality improvement initiatives that align with OhioRISE program requirements.
• Support the development of the local system of care serving youth and families.

Qualifications of a Successful Manager of OhioRISE Care Coordination/Case Management:

• Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or higher.
• Demonstrated supervisory or managerial experience in community-based services.
• Bring a minimum of 3 years of experience in children's mental health or related public sectors, providing community-based services to children, youth, families, or caregivers.
• Display experience in managing complex cases involving multi-system youth.
• Monitor timely and accurate documentation and coach staff to performance.
• Maintain an outstanding driving record.
• Ability to obtain NPI and Ohio Medicaid numbers (assistance available).
