Job summary

Group Health Cooperative of Eau Claire is seeking a dedicated healthcare professional to plan, direct, and evaluate managed care options for members within an interdisciplinary team. The role emphasizes continuity of care, confidentiality, and collaboration with providers and community partners.

Minimum qualifications include an associate degree in social work, human services, or nursing and an active Wisconsin license.

Qualifications

• Associate degree in social work, human services or nursing.
• Active Wisconsin RN, LPN, or Social Work license required.
• D-SNP Case Managers require active Wisconsin RN license.
• Strong customer service and written/verbal communication skills.

Responsibilities

• Initiate and implement a managed health care plan for members and provide support and tools.
• Assess social and emotional needs and foster member independence.
• Advocate for access to services and positive member outcomes.
• Identify problems and provide effective solutions.
• Ensure accurate documentation and reporting of care management activities.
• Collaborate with coworkers and external resources for continuity of care.
• Attend professional trainings and stay up to date with changes in discipline.
