Who We’re Looking For

Copa Health is seeking a Housing Specialist to join our Medically Assertive Community Treatment (MACT) Team. The MACT Housing Specialist provides direct treatment, rehabilitative and supportive services in a community setting to consumers. They work as a member of a clinical team to assess consumer needs and wants, participate in the development of individual service plans, act as a consumer advocate and liaison with families and community agencies. The role includes on‑call crisis intervention services, weekend and holiday coverage, and serves as the MACT’s specialist on housing, collaborating with other MACT members to provide housing‑related services for MACT consumers. A flexible work schedule is required.

Key Responsibilities
  • Work as part of a clinical team to facilitate recovery and inspire hope for consumers while supporting their achievement of service plan goals. Provide ongoing assessment and feedback on progress in relation to the service plan, regardless of the consumer’s current location.
  • Meet with consumers in the community in accordance with established policies, procedures, and clinical protocols to provide supportive counseling skill development and other support services as needed.
  • Work with a consumer’s Prescribing Clinician, Nurse, and other team members to ensure timely development and implementation of the individual’s service plan (including all necessary assessments) and crisis plan.
  • Coordinate with all parties involved in a consumer’s treatment according to policy, procedure, and clinical protocols, including inter‑ and intra‑agency staffing, communication by telephone and letter, and service/support delivery as indicated on the service plan.
  • Transport consumers to necessary services as outlined in their service plan, as needed.
  • Assume “primary contact” responsibility for a percentage of team consumers to maintain accurate, thorough, and current documentation in the medical record, keep weekly client schedules updated, and act as first contact for those consumers and their families.
Qualifications
  • Combination of education and years of experience (see options below):
  • Master’s degree in a behavioral health related field.
  • Bachelor’s degree in a behavioral health related field (required) with two years of behavioral health work experience (preferred).
  • Associate degree in a behavioral health related field with four years of behavioral health work experience.
  • High school diploma or equivalent with four years of behavioral health work experience.
  • Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.
  • Ability to meet Copa’s driver authorized driving requirements, which include an Arizona driver’s license upon hire, a 39‑month MVR, proof of insurance, and vehicle registration (if applicable).
