Job summary

Diverge Health is seeking an Integration Project Manager in Chicago to lead a multi-phase merger integration. You will bridge strategy and execution, acting as the operational backbone to keep workstreams aligned and executives informed.

You will build detailed 2026 plans, manage risks, and drive change management across both organizations, partnering with leadership to shape the 2027 roadmap while ensuring disciplined, human-focused integration.

Qualifications

• PMP certification or equivalent project management credential.
• 3+ years of project management experience, including multiple end-to-end M&A integrations.
• Proficiency in modern PM tooling: Aha!, Microsoft Project, and SharePoint.
• Comfort with high-availability expectations, including evenings and weekends for escalations.
• Healthcare industry experience, particularly in value-based care, tech-enabled clinical services, FQHCs, or managed care.

Responsibilities

• Inherit an early-stage merger integration and carry it forward with detailed execution.
• Serve as the operational backbone, keeping workstreams on track and executives aligned.
• Build detailed 2026 execution plans with clear ownership and milestones.
• Track status, surface risks, and drive decisions with stakeholders.
• Build and execute a change management strategy across both organizations.
• Deliver executive-ready updates for steering committee reviews and leadership.
