Job summary

All About Kids Home Health is seeking a Licensed Vocational Nurse (LVN) for 1:1 care in patients' homes in the San Juan area. Weekend night shifts are available; candidates with Trach/GJ-Tube experience and willingness to travel to clients’ residences are preferred.

Must have LVN license and BLS certification, strong computer skills, and the ability to lift and transfer patients. Home health environment; English/Spanish language skills appreciated.

Qualifications

• For 1:1 care in patient homes.
• Trach / GJ-Tube experience is required.
• Must have computer skills and reliable transportation.
• Must be able to stoop/bend and lift/transfer clients.
• Must read 12 point font and work in home health settings.
• Home-based care for medically fragile children.

Responsibilities

• Provide direct nursing care in patients' homes.
• Monitor and manage medical equipment (GJ-tube, Trach, Vent) as trained.
• Coordinate with families and care teams for therapy plans.
• Ensure safety and quality of care during visits.

All About Kids Home Health services children from the ages of 0 to 21yrs. who are considered medically fragile. We offer Private Duty Nursing, Skilled Nurse Visits, Physical Therapy, Occupational Therapy and Speech Therapy.
