Responsibilities include coordinating discharge, discussing post-care options with patients and families, maintaining accurate records, and establishing referrals with providers to ensure seamless transitions of care.

Qualifications

• Knowledge of confidentiality and HIPAA regulations.
• Experience coordinating discharge planning and continuing care.

Responsabilités

• Collaborates with physicians, therapists, treatment team, and patient to develop effective continuing care plans.
• Makes telephone calls, faxes, copies and coordinates discharge plans.
• Provides patients with aftercare options and discusses options with patient and/or family.
• Collects releases of information to research and refer to services.
• Maintains timely and accurate records.
• Initiates contact with referral sources and community providers to facilitate continuity of care.
• Coordinates admissions to secondary hospitals, treatment centers, nursing homes, and other placements.
• Schedules follow-up appointments with psychiatrists and therapists as outpatient care continues.
• Researches and updates provider and treatment facility lists.
• Maintains an up-to-date referral, contact, and resource database.
