Monterey Park Hospital seeks a Case Manager for the Utilization Review Department. This is a full-time, 8-hour day shift role from Saturday through Wednesday, reporting to the Chief Operating Officer.

The Case Manager will coordinate Continuum of Care from admission to discharge, initiate admission reviews, prioritize continue stays, and review post-hospital plans with the physician and care team. California RN or CA LVN license is required, with UR experience preferred.

Qualifikationen

• RN license required (California) or CA LVN license.
• 3–5 years of case management (Utilization Review) experience preferred.
• Bachelor's degree preferred; BLS certification preferred.

• Initiate admission review and prioritize continue stay reviews.
• Review post-hospital care plans with physicians, family and care team.
• Process TAR submissions and assess medical necessity per Interqual/MCG guidelines.
• Coordinate discharge planning and communicate with health plans and care managers.
