Job summary

White Blossom Care Center in San Jose, CA is seeking an experienced LVN Supervisor to join our leadership team. This role oversees nursing operations on assigned shifts, providing clinical oversight and mentorship to LVNs and CNAs.

The ideal candidate holds an active California LVN license, SNF experience, and proficiency with PCC (PointClickCare). You will ensure regulatory compliance, admissions coordination, care plan updates, and staffing support.

Qualifications

• Active California LVN license in good standing.
• Direct SNF experience required; 1-2 years of supervisory leadership preferred.
• Proficiency with PCC audits and clinical documentation.
• Strong understanding of Title 22 and long-term care regulations.
• Excellent communication, leadership and organizational abilities.

Responsibilities

• Leadership: Supervise and support nursing staff (LVNs and CNAs), providing guidance and feedback.
• Clinical Oversight: Manage resident admissions and related clinical documentation.
• Quality Assurance: Conduct PCC audits to ensure accuracy and compliance.
• Compliance: Ensure adherence to state, federal, and facility regulations.
• Collaboration: Update care plans with the interdisciplinary team; monitor resident conditions.
• Operations: Assist with staffing, scheduling, and maintaining a safe, compassionate environment.
