Job summary

White Blossom Care Center in San Jose, CA is seeking an experienced LVN Supervisor to join our leadership team. This role focuses on clinical oversight, guiding frontline staff, and ensuring regulatory compliance for resident care.

You will lead admissions, conduct audits, and mentor staff across shifts in a fast-paced SNF setting, with a strong emphasis on Title 22 compliance and quality care standards.

Qualifications

• Active California LVN license in good standing.
• Direct SNF experience required and leadership exposure.
• Proficiency with PCC audits and clinical documentation.
• Strong knowledge of Title 22 and long-term care regulations.

Responsibilities

• Oversee nursing operations on assigned shifts, ensuring resident care and regulatory compliance.
• Supervise LVNs and CNAs, provide guidance and feedback.
• Coordinate admissions and complete clinical documentation.
• Perform PCC audits and monitor care plan updates with the interdisciplinary team.
