Job summary

Rockport Healthcare Services in Oxnard, CA seeks a licensed practical nurse under the supervision of a Registered Nurse to provide nursing care in a long-term care setting. You will administer medications, document care, and participate in care planning and rounds to support residents’ highest practicable well-being.

Requirements include graduation from an accredited nursing program, current California license in good standing, CPR certification, and strong communication skills.

Qualifications

• Graduate of an accredited school of nursing or approved LVN program.
• Current nursing licensure in good standing with the State licensing board.
• Possess the willingness to work with professional and non-professional personnel. Proficiency of the English language including the ability to speak, read, write, and comprehend the language with minimal to no difficulty.
• Valid CPR certification.

Responsibilities

• Provides nursing care as prescribed by physician/health care professional in accordance with the legal scope of practice.
• Administers professional services and provides care to attain residents' highest practicable physical, mental, and emotional well-being.
• Admits, transfers, and discharges residents in accordance with policy and procedure.
• Orients new residents/families as necessary.
• Maintains a clean and safe unit for resident comfort and convenience.
• Conducts daily resident rounds to observe physical and emotional status.
• Records care information accurately and completes required documentation in the medical record.
• Assists in developing and updating resident Plans of Care as indicated.
• May administer medications and reports significant changes in condition to physicians.
• Notifies physicians and documents incidents per policy; participates in care conferences and rounds.
• Supervise CNAs and assist in the supervision of nursing personnel.
• Participates in infection control and QA activities as requested.
