Job summary

Tice Valley Post Acute is seeking a compassionate Licensed Vocational Nurse (LVN) to join our team on a PRN basis. This flexible opportunity focuses on resident-centered care in a skilled nursing and post-acute setting.

The LVN will work with the interdisciplinary team to deliver safe, compassionate care, with duties including medication administration, assessments, documentation, and ongoing resident monitoring. California LVN license required.

Qualifications

• Current California LVN license in good standing.
• Previous skilled nursing or post-acute experience preferred.
• Strong clinical, organizational, and communication skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Provide nursing care to residents in accordance with physician orders and care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and report changes to the appropriate healthcare professionals.
• Complete accurate and timely nursing documentation.
• Assist with assessments, care planning, and ongoing resident monitoring.
• Communicate effectively with residents, families, physicians, and members of the healthcare team.
• Follow facility policies, procedures, and applicable state and federal regulations.
• Maintain a safe, clean, and supportive environment for residents.
• Respond appropriately to resident needs and changes in condition.
• Promote resident dignity, comfort, and quality of life.
