Humana is recruiting for a Patient Services Coordinator, LPN to ensure care coordination and scheduling across home health teams. The role supports field staff, patients, physicians, and office operations, with duties including schedule management, reports, and occasional clinical tasks.

The ideal candidate is a licensed RN or LVN with at least 1 year of home health experience and strong communication and organizational skills. A valid driver’s license is required.

Qualifications

• Must be licensed as a Professional Nurse or Licensed Vocational Nurse in the state of practice.
• Minimum 1 year of home health experience preferred.
• Valid state driver's license and automobile liability insurance required.
• Excellent communication, organization, and multitasking skills.

Responsabilités

• Manage schedules for all patients and edit schedules for staff absences.
• Coordinate with field staff, patients, physicians and office personnel.
• Assist with day-to-day office and staff management.
• Run scheduling reports and prepare weekly staff schedules.
• Assist with clinical tasks such as receiving lab reports and routing to appropriate staff.
• Participate in on-call rotation and may perform patient visits as needed.
