Job summary

East Bay Post Acute is seeking a compassionate Licensed Vocational Nurse (LVN) to join our clinical team part-time in East Bay Post Acute. The LVN will deliver direct nursing care, administer medications, monitor residents, and collaborate with the care team to ensure quality outcomes and resident well-being.

The role requires an active California LVN license, CPR/BLS certification, and prior post-acute care experience is preferred.

Responsibilities

• Provide direct nursing care in accordance with physician orders and individualized care plans.
• Administer medications, treatments, and nursing procedures safely and accurately.
• Monitor residents' conditions and promptly report changes to physicians and nursing leadership.
• Complete accurate and timely clinical documentation.
• Assist with resident admissions, transfers, and discharges.
• Collaborate with CNAs, RNs, therapists, physicians, and other healthcare professionals to coordinate resident care.
• Follow facility policies, infection control procedures, and regulatory requirements.
• Provide compassionate support, education, and communication to residents and their families.
• Promote resident safety, dignity, comfort, and quality of life.
