As an LVN, you will play an important role in delivering quality nursing care, supporting resident wellness, and working alongside CNAs, RNs, physicians, and other healthcare professionals to ensure excellent resident outcomes.
Position: Licensed Vocational Nurse (LVN)
Status: Part-Time
Pay Rate: $34.00–$35.00 per hour
Location: East Bay Post Acute
Provide direct nursing care in accordance with physician orders and individualized care plans.
Administer medications, treatments, and nursing procedures safely and accurately.
Monitor residents' conditions and promptly report changes to physicians and nursing leadership.
Complete accurate and timely clinical documentation.
Assist with resident admissions, transfers, and discharges.
Collaborate with CNAs, RNs, therapists, physicians, and other healthcare professionals to coordinate resident care.
Follow facility policies, infection control procedures, and regulatory requirements.
Provide compassionate support, education, and communication to residents and their families.
Promote resident safety, dignity, comfort, and quality of life.
Current California LVN license in good standing.
Current CPR/BLS certification.
Previous skilled nursing, rehabilitation, or post-acute care experience preferred.
Strong clinical assessment and critical-thinking skills.
Excellent communication and interpersonal abilities.
Ability to work effectively in a fast-paced, team-oriented environment.
