Job summary

East Bay Post Acute in the East Bay area is seeking a Licensed Vocational Nurse (LVN) for a Full-Time AM shift to join our dedicated clinical team. You will provide compassionate, resident-centered care and support to our residents.

Responsibilities include direct nursing care per physician orders, administering medications and treatments, monitoring residents, documenting promptly, and collaborating with CNAs, RNs, therapists, and physicians to ensure safe, high-quality outcomes while

Qualifications

• Current California LVN license in good standing.
• CPR/BLS certification.
• Experience in skilled nursing or post-acute care preferred.
• Strong clinical assessment and critical-thinking skills.

Responsibilities

• Provide direct nursing care in accordance with physician orders and individualized care plans.
• Administer medications, treatments, and nursing procedures safely and accurately.
• Monitor residents' conditions and promptly report changes to physicians and nursing leadership.
• Complete accurate and timely clinical documentation.
• Assist with resident admissions, transfers, and discharges.
• Collaborate with CNAs, RNs, therapists, physicians, and other healthcare professionals to coordinate resident care.
• Follow facility policies, infection control procedures, and regulatory requirements.
• Provide compassionate support, education, and communication to residents and their families.
• Promote resident safety, dignity, comfort, and quality of life.
