Job summary

East Bay Post Acute in California is seeking a compassionate Licensed Vocational Nurse (LVN) to join our clinical team on a part-time basis. You will provide direct nursing care, administer medications, and collaborate with CNAs, RNs, and physicians to support resident wellness.

Qualifications

• Current California LVN license in good standing.
• Current CPR/BLS certification.
• Previous skilled nursing, rehabilitation, or post-acute care experience preferred.

Responsibilities

• Provide direct nursing care in accordance with physician orders and individualized care plans.
• Administer medications, treatments, and nursing procedures safely and accurately.
• Monitor residents' conditions and promptly report changes to physicians and nursing leadership.
• Complete accurate and timely clinical documentation.
• Assist with resident admissions, transfers, and discharges.
• Collaborate with CNAs, RNs, therapists, physicians, and other healthcare professionals to coordinate resident care.
• Follow facility policies, infection control procedures, and regulatory requirements.
• Provide compassionate support, education, and communication to residents and their families.
• Promote resident safety, dignity, comfort, and quality of life.
