Job summary

Bristol Hospice in Modesto, CA is seeking a compassionate Licensed Practical Nurse (LPN/LVN) to provide direct patient care in end-of-life settings. The role requires CA licensure and a minimum of three years nursing experience, preferably in hospice or medical/surgical areas.

The position emphasizes patient and family education, care coordination, accurate documentation, and adherence to hospice philosophies. Flexibility in hours and strong information-system skills are essential.

Qualifications

• Must currently be licensed as an LPN/LVN in the State.
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice and the needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate willingness to maintain knowledge of information systems and software programs

Responsibilities

• Provide direct patient care per State Nurse Practice Act, including end-of-life care
• Implement current nursing practice following the plan of care
• Document patient services accurately and timely
• Educate patients and families about diagnosis and self-care
• Coordinate hospice services and report needs to the interdisciplinary team
• Prepare equipment and supplies per aseptic technique
• Manage pain and symptoms
• Assess patient responses to treatments and medications
• Participate in ongoing professional development and quality improvement

Qualifications: An Average Day:

(Includes, but not limited to)

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
