Bristol Hospice in Atlanta, GA is seeking a compassionate Licensed Vocational Nurse (LVN/LPN) to join our hospice team. The role focuses on direct patient care, end-of-life support, and collaboration with an interdisciplinary group to deliver dignified, patient-centered hospice services.

The Daily duties include care planning, documentation, family education, and coordination with therapy and social work teams.

Qualifications

  • Must be licensed as an LPN/LVN in the state.
  • Minimum of 3 years nursing experience; hospice or medical/surgical preferred.
  • Understand hospice philosophy and care for terminally ill patients.
  • Must be flexible in work hours.
  • Maintain knowledge of information systems and software used.
  • Provide patient education and coordinate with the team.

Responsibilities

  • Provide direct patient care per State Nurse Practice Act with hospice-focused duties.
  • Implement assessments and plan of care.
  • Document patient services accurately.
  • Educate patients and families about diagnosis and care.
  • Coordinate hospice services with interdisciplinary team.
  • Prepare and use equipment; adhere to aseptic technique.
  • Manage pain and symptoms; monitor treatment responses.
  • Participate in ongoing professional development and quality initiatives.

Qualifications

An Average Day:

(Includes, but not limited to)

  • Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique
  • Provide appropriate pain/symptom management
  • Evaluate patients response to treatments/medications
  • Participate in personal, professional growth and development
  • Participate in organization's quality assessment performance improvement teams and activities
  • Perform other duties as assigned by the registered nurse
  • All other duties as assigned
Requirements
  • Must currently be licensed as an LPN/LVN in the State
  • Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
  • Must understand the philosophy of hospice concept and understand needs of the terminally ill
  • Must be flexible in work hours
  • Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
