Job summary

Bristol Hospice - Green Bay Llc is seeking a compassionate Licensed Vocational Nurse (LVN) to join our Green Bay team dedicated to high-quality end-of-life care with dignity and respect.

You will provide direct patient care under state guidelines, document care plans accurately, educate patients and families, and coordinate hospice services with an interdisciplinary team to ensure comfort and quality of life for our patients.

Qualifications

• Must currently be licensed as an LPN/LVN in Wisconsin.
• Three years nursing experience preferred (community health/hospice or medical/surgical).
• Understand hospice philosophy and care for terminally ill patients.
• Flexible in work hours and availability.
• Willing to maintain knowledge of information systems and software.

Responsibilities

• Provide direct patient care as defined by State Nurse Practice Act.
• Document accurately and timely to reflect plan of care.
• Educate patient and family/caregiver about diagnosis and self-care.
• Coordinate hospice services with interdisciplinary team.
• Prepare and use equipment with aseptic technique; manage pain and symptoms.
• Evaluate patient responses to treatments/medications; participate in QA teams.
