Job summary

Bristol Hospice in Merced, CA is seeking a compassionate Licensed Vocational Nurse (LVN/LPN) to join our PRN hourly team delivering end-of-life care.

Provide direct patient care, follow care plans, document services, and educate patients and families. Work with hospice team to meet patient needs.

Flexible weekend schedule, weekend availability preferred; must hold CA LPN/LVN license and have strong commitment to hospice philosophy.

Qualifications

• Must be licensed as an LPN/LVN in California.
• Minimum 3 years of nursing experience.
• Hospice knowledge and philosophy understood.
• Flexibility in work hours.
• Willingness to maintain knowledge of information systems/software.

Responsibilities

• Provide direct patient care per State Nurse Practice Act in end-of-life settings.
• Implement care plans based on comprehensive assessments.
• Document patient services accurately and timely.
• Educate patients and families about diagnosis and self-care.
• Coordinate hospice services with interdisciplinary team.
• Prepare equipment and supplies adhering to aseptic technique.
• Manage pain and symptom control.
• Participate in professional development and quality improvement.
• Perform other duties as assigned by RN.
• All other duties as assigned.

Qualifications: An Average Day:

(Includes, but not limited to)

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
