Sweetwater Care is seeking an experienced Licensed Vocational Nurse/Licensed Practical Nurse to provide nursing care within the scope of practice and in alignment with standards of care, physician orders, and local regulations.

The role includes charting, observing patients, and collaborating with physicians; nursing management experience is preferred but not required. Excellent adherence to policies and patient safety is essential.

Qualifikationen

• Licensed LVN/LPN in the state of practice.
• Long-term care nursing experience.
• Experience with PointClickCare charting.
• BLS/CPR certification.
• Nursing management experience preferred but not required.

Aufgaben

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutrition, infections, skin conditions, weight loss or other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

Kenntnisse
