Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within scope of practice in a long-term care setting. The role follows physician orders and center policies, ensuring standards of care and patient needs are met.

Qualifications

• Long Term Care experience required.
• Experience with Point-click care charting.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is a plus.

Responsibilities

• Follows established charting procedures and incident reporting.
• Manages narcotics counting, pharmacy orders, and restraint reduction per policy.
• Document changes in patient conditions (nutrition, infections, skin issues, weight loss, etc.).
• Utilize the Quality Life Manual to identify, place, and monitor patients in programs.
• Document and notify departments of room changes, LOA, admits, or discharges.
• Makes rounds with physicians when necessary.
