Job summary

Sweetwater Care in California seeks a dedicated Licensed Vocational Nurse (LVN/LPN) to provide nursing care within scope of practice, aligned with physician orders and center policies.

Responsibilities include charting, incident reporting, participating in rounds with physicians, and monitoring changes in patients' conditions while following narcotic and restraint procedures, to support quality care and patient safety.

Qualifications

• Requires LVN/LPN license in practicing state.
• BLS/CPR certification required or willingness to obtain.
• Experience in long-term care and charting software preferred.
• Nursing management experience is a plus but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
