Job summary

Transition Home Healthcare in Houston, TX is seeking a licensed vocational nurse (LVN) to join our elite home health team. The role includes delivering patient-centered home health visits, coordinating with a multidisciplinary team, and documenting care accurately.

Shifts available PRN and Full Time, serving Southwest and Inner Loop areas. We offer competitive pay, mileage reimbursement, and a comprehensive benefits package including medical, dental, vision, life, and 401(k) with employer match.

Qualifications

• Valid Texas LVN license (or compact state).
• Minimum 1 year of home health experience.
• Knowledge of home health regulations and compliance.

Responsibilities

• Collaborate with the home healthcare team to develop and implement individualized care plans.
• Complete home care nursing visits.
• Prepare timely, complete, and accurate clinical notes.

Responsibilities:

• Collaborate with the home healthcare teamto develop and implement individualized care plans.
