Transition Home Healthcare is a premier home health agency serving Houston, TX and surrounding areas since 2010. We seek an LVN/LPN to join our team for PRN and full-time shifts in home health care, delivering patient-centered nursing visits and collaborative care planning.

Responsibilities include completing visits, documenting accurately, and educating patients and families. Familiarity with HCHB EMR is a bonus; bilingual Spanish is encouraged to apply.

Qualifications

• Active LVN/LPN license in Texas or compact state.
• Minimum 1 year experience in home health.
• Bilingual (Spanish) encouraged to apply.
• Knowledge of home health regulations and compliance.

Responsibilities

• Collaborate with the home health care team to develop and implement care plans.
• Complete home care nursing visits.
• Prepare timely, accurate clinical notes.
• Communicate with healthcare providers and care team members.
• Educate patients and families about health conditions and treatment plans.
• Adhere to agency policies, procedures, and regulatory requirements.

LVN or LPN (Home Health)

Shifts Available: PRN & Full Time

Service Areas for Coverage:

• Northwest (Spring/Cypress,Tomball, Willowbrook, Inwood, Heights, Jersey Village)
• Northeast (Crosby,Atascocita, Kingwood, Humble)
• Southwest (59-288)

Responsibilities:

• Collaborate with the home healthcare teamto develop and implement individualized care plans.
• Complete home care nursing visits.
• Prepare timely, complete, and accurate clinical notes.
• Communicate with healthcare providers and other care team members.
• Educate patients and their families about health conditions, treatment plans, and self-care techniques.
• Adhere to all agency policies, procedures, and regulatory requirements related to home health care.

Qualifications:

• Valid and current LVN or LPN license in the state of Texas or compact state.
• Minimum of one (1) year of experience in a home health setting.
• Bilingual (Fluent in Spanish) encouraged to apply
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred but not required)
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
