The LVN Intake Coordinator is responsible for taking all hospice information calls regarding the intake of new referrals to facilitate an evaluation for hospice service. The LVN manages all patient intake functions throughout the entire intake process and provides hospice evaluations as directed.
$35-38 hourly.
