Job summary

Health & Human Services Commission is seeking a Licensed Vocational Nurse II to provide skilled nursing care at the Lone Pine - Nursing unit within the Lufkin State Supported Living Center. This role supports medically fragile individuals across morning, evening, and night shifts, with options for overtime and on-call rotations.

Qualifications

• Graduation from an LVN program.
• Texas LVN license or reciprocal eligibility.
• Experience with electronic health records preferred.
• CPR certification required.
• Willing to work weekends/holidays.
• Ability to serve individuals with intellectual/developmental disabilities.

Responsibilities

• Performs complex and technical LVN nursing duties for medically fragile individuals.
• Administers medications and treatments per physician orders.
• Maintains accurate health records in the electronic health record system.
• Trains and mentors new LVN staff and direct care team.
• Collaborates with RNs, physicians, and other staff to implement care plans.
• Ensures compliance with HHSC and SSLC policies and safety procedures.
