Job summary

Angels Care Home Health is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients’ homes under RN supervision. The LVN will coordinate care with the interdisciplinary team, patients, families, and referring agencies to ensure quality, compassionate care.

Qualifications

• Graduate of an accredited school of vocational/practical nursing.
• Current state license as an LVN.
• Current State’s Driver’s License.
• Two years’ experience as an LVN, home health preferred.
• Proof of current CPR and Hepatitis profile

Responsibilities

• The LVN administers skilled nursing care under the supervision of a registered nurse in clients’ homes.
• Coordinates care with the interdisciplinary team, patient/family, and referring agency.
