Job summary

Angels Care Home Health is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care in clients' homes under the supervision of a registered nurse. The position requires coordination with the interdisciplinary team and communication with patients, families, and referring agencies.

Qualified candidates have an LVN license, a valid driver’s license, CPR and hepatitis profile, and at least two years of LVN experience in home health.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current state license as an LVN.
• Current State’s Driver’s License.
• Two years’ experience as an LVN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Primary function of the LVN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
