Job summary

Angels Care Home Health is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients' homes under the supervision of a registered nurse. The role involves coordinating with an interdisciplinary team, patients, families, and referral sources.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Two years’ LVN home health experience preferred.
• Current LVN license in the state.
• Proof of CPR and Hepatitis profile up to date.
• Valid Texas driver’s license.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
• Assess patient needs and develop care plans.
• Maintain accurate and timely documentation.
