Job summary
AltaMed Health Services Corp. in Los Angeles, CA is seeking a Licensed Vocational Nurse, Home Care Coordinator to develop and implement homecare services for program participants, coordinating contracted home services, DME, and nursing home care.
You will attend IDT and coordination meetings to ensure seamless patient care. The ideal candidate has LVN licensure in California, at least one year of LVN experience, and a valid driver’s license with a good record; BLS certification is required or
Qualifications
- Graduation from an accredited LVN school with a California LVN license.
- Minimum one year of LVN nursing experience.
- Experience with physical, mental, and social needs of patients.
- At least one year serving frail or elderly populations.
- Valid California driver’s license with good driving record.
- BLS certification or higher per AHA/Red Cross guidelines.
Responsibilities
- Develop and implement homecare services for program participants.
- Coordinate contracted home services, durable medical equipment and nursing home care.
- Attend scheduled IDT and coordination meetings.
