AltaMed Health Services seeks an LVN to develop and implement home care services for participants in our program. You will coordinate contracted home services, equipment, and nursing home care, while attending scheduled IDT meetings.

The role requires an active CA LVN license and at least 1 year of LVN experience, including experience with frail or elderly populations. A valid CA driver’s license and BLS certification are also required.

Qualificações

• Graduation from an accredited LVN school and a current LVN license issued by the State of California.
• Minimum of one (1) year of experience as a Licensed Vocational Nurse.
• Experience and knowledge regarding the physical, mental, and social needs of patients.
• At least one year of working with a frail or elderly population.
• Valid California driver’s license with a good driving record.
• Minimum requirement of a valid BLS certification or higher per AHA or Red Cross guidelines.

Responsabilidades

• Develop and implement homecare services for program participants, coordinating all contracted home services, durable medical equipment, and nursing home care.
• Attend scheduled IDT and Coordination meetings.
