Job summary

East Bay Post Acute is hiring a Licensed Vocational Nurse (LVN) for a full-time AM shift in California. The LVN will provide direct nursing care, administer medications, monitor residents, and document care plans while collaborating with the care team to ensure quality outcomes.

Requirements include a CA LVN license in good standing and CPR/BLS certification; experience in SNF/post-acute settings is preferred. Competitive hourly pay and a supportive culture are offered.

Qualifications

• Current California LVN license in good standing.
• CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, or post-acute care preferred.
• Excellent communication and interpersonal abilities.

Responsibilities

• Provide direct nursing care in accordance with physician orders and individualized care plans.
• Administer medications, treatments, and nursing procedures safely and accurately.
• Monitor residents' conditions and promptly report changes to physicians and nursing leadership.
• Complete accurate and timely clinical documentation.
• Assist with resident admissions, transfers, and discharges.
• Collaborate with CNAs, RNs, therapists, physicians, and other healthcare professionals to coordinate resident care.
• Follow facility policies, infection control procedures, and regulatory requirements.
• Provide compassionate support, education, and communication to residents and their families.
• Promote resident safety, dignity, comfort, and quality of life.
