Job summary

Addus in Texas seeks an LVN Field Supervisor to conduct on-site client evaluations and develop service plans in coordination with authorization, and to perform other field visits as assigned in Ararillo, TX.

You will visit as scheduled, report daily to the scheduler, recruit, hire, train, and supervise attendants, and ensure forms are completed and turned in within 48 hours of each visit.

Qualifications

• Two years full-time study at an accredited college or university or a high school diploma with substitute supervisory experience.
• PHC, MC experience preferred

Responsibilities

• Perform on-site client evaluations and develop service plans with authorization.
• Complete visits as scheduled and report daily to scheduler.
• Recruit, hire, train, and supervise attendants.
• Complete and attach required forms to daily visit reports and submit within 48 hours of visit.
