Job summary
Wellpath / Correct Care Solutions is seeking a Nurse Discharge Planner LVN in California to develop and coordinate clinically informed reentry care plans for CalAIM-eligible individuals. The role includes basic nursing assessments, health record reviews, referrals, and warm handoffs to managed care plans and community providers.
The position requires current LPN/LVN licensure, CPR, and the ability to obtain facility security clearance.
Qualifications
- Current licensure as a Licensed Practical/Licensed Vocational Nurse.
- Bachelor's degree in Social Work, Business Administration, or other health-related fields is preferred.
Responsibilities
- Provide discharge planning services for medical, mental health, MAT, and related programs.
- Identify release-planning needs and coordinate medications and medical needs upon release.
- Arrange referrals and appointments with community providers.
- Facilitate psychoeducational sessions, benefit applications, and resumption of services.
- Maintain relationships with facility staff, parole, mental health, and medical partners while adhering to security regulations.
