Responsibilities
- Develop and coordinate reentry care plans for patients.
- Conduct basic nursing assessments and review health records.
- Ensure timely service delivery by documenting in EHR and tracking release timelines.
- Facilitate warm handoffs to community providers.
- Maintain relationships with facility staff and external partners.
