Job summary

Sweetwater Care seeks an experienced Licensed Vocational Nurse (LVN) in California to lead nursing operations on a shift and support patient care within policy and state guidelines.

You will administer medications and treatments timely, monitor patient changes, and coordinate with physicians, families, and other departments. Prior SNF/LTC management experience is preferred, and bilingual skills are a plus for diverse resident needs.

Qualifications

  • Current California LVN license.
  • Minimum 1 year nursing management experience preferred.
  • SNF/LTC experience preferred.
  • Bilingual abilities a plus.

Responsibilities

  • Provides nursing care within scope of practice and adheres to standards and policies.
  • Participates in quality improvement and infection control.
  • Administers medications and treatments per policy and orders.
  • Notifies departments of patient changes and educates patients/families.
  • Assists physicians during rounds when needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient's needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities
  • Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
  • Understands, participates and assists in implementing the Quality Improvement Program.
  • Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
  • Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
  • Conducts family and patient education.
  • Demonstrates the ability to administer medications timely and according to facility policy.
  • Demonstrates the ability to administer treatments timely and according to facility policy.
  • Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
  • Makes rounds with physicians when necessary.
Qualifications
  • Currently licensed as LVN in state California.
  • One-year prior nursing management experience preferred.
  • SNF/LTC experienced preferred.
  • Bilingual is a plus.
