Job summary

Sweetwater Care is seeking an on-site LVN with management experience in a California long-term care setting. The role focuses on delivering nursing care within scope, implementing physician orders, and ensuring patient needs are met in line with policies and guidelines.

The ideal candidate has prior nursing management experience, familiarity with long-term care operations, and proficiency with PointClickCare charting to support timely care and documentation.

Qualifications

• Licensed LVN in California or with a compact license if working in a compact state.
• One-year nursing management experience preferred.
• Long-term care experience is required.

Responsibilities

• Provides nursing care within the scope of practice and as per standards and physician orders.
• Demonstrates knowledge and use of the Infection Control Manual.
• Participates in the Quality Improvement Program and uses the Quality Life Manual.
• Notifies other departments of patient changes such as room changes, admits and discharges.
• Educates patients and families in care plans and procedures.
• Administers medications and treatments per facility policy and physician orders.
• Receives, transcribes, and carries out physician orders within Nurse Practice Act.
• Participates in rounds with physicians as needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
