Job summary

Sweetwater Care is seeking a licensed LVN/LPN in Colorado to manage nursing care within the facility’s policies and physician orders. You will participate in infection control and quality improvement initiatives, educate patients and families, and ensure timely administration of medications and treatments.

The role requires Colorado licensure, with nursing management experience preferred and long-term care exposure.

Qualifications

• Licensed LVN/LPN in Colorado
• Nursing management experience preferred, not required
• Long-term care experience
• Point Click Care charting experience

Responsibilities

• Follows Infection Control Manual and quality program procedures
• Participates in Quality Improvement activities and Life Quality program
• Notifies departments of patient changes (room changes, LOA, admits, discharge)
• Educates patients and families
• Administers medications and treatments per facility policy
• Transcribes and carries out physician orders within scope
• Assists in rounds with physicians as needed

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN/ LPN in state
• One-year prior nursing management experience preferred, Not Required
• LONG TERM CARE EXPERIENCE
