Position: Licensed Vocational Nurse (LVN) Coordinator

MLK Community Healthcare is seeking a Licensed Vocational Nurse (LVN) Coordinator to be part of our Care Management program. The LVN Coordinator will coordinate care for patients enrolled in Enhanced Care Management (ECM) and Disease Management programs. The role involves participating in interdisciplinary care team meetings, conducting nursing assessments, developing care plans, and working with the medical team to support patient health goals. This position requires self-motivation, clinical expertise, and patient engagement skills. The LVN Coordinator will collaborate with various partner organizations to provide transitional healthcare support.

Essential Duties and Responsibilities
  • Provide clinical assistance for ECM and Disease Management to address medical and behavioral health needs.
  • Develop and evaluate patient-centered care plans, ensuring timely follow-up and closing care gaps.
  • Assist in identifying health care needs and interventions tailored to individual patients.
  • Coordinate with healthcare providers to ensure comprehensive care and support medication adherence.
  • Review medical summaries and prepare for appointments in collaboration with Care Managers.
  • Train staff on documentation, quality measures, and EHR system (Cerner) utilization.
  • Engage vulnerable populations through outreach, home visits, and community interactions.
  • Address Social Determinants of Health and connect patients with community resources.
  • Coordinate hospital admission/discharge plans to prevent readmissions.
  • Perform medication reconciliation and support adherence.
  • Assist with billing and health plan chart audits.
  • Travel to MLKCH sites as needed to support patient care.
  • Perform other duties as assigned.
Position Requirements
  • High School diploma or GED.
  • Minimum one year of experience in a medical practice, healthcare, or hospital setting.
  • Six months of project coordination experience preferred.
  • Valid CA Driver’s License with insurance proof (must present within 30 days).
  • Current BLS certification from the American Heart Association.
  • Current LVN license in California.
  • Care Management experience preferred.
  • Strong organizational, communication, and decision-making skills.
  • Experience working with underserved populations and understanding homelessness is preferred.
  • Ability to coordinate care effectively and work independently.
