Job summary

PACS is hiring for an LVN to provide direct nursing care to residents in a Porterville SNF setting. You will serve as Charge Nurse on the NOC shift, supervise CNAs, administer medications, and monitor resident conditions. Strong communication and EMR proficiency are essential; new grads are welcome as we offer training.

Qualifications

• Current California LVN license in good standing.
• CPR certification required.
• Experience in a SNF or post-acute setting preferred, but not required.
• New graduates encouraged to apply – we will train you!
• Strong written and verbal communication skills.
• Computer literate and comfortable with EMR systems.
• Compassionate, dependable, and a team player.
• Must be able to provide recent medical clearance to work in SNF.
• Must pass background check and be eligible to work in the U.S.

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans
• Serve as the Charge Nurse during NOC shift, overseeing CNAs and ensuring high-quality resident care
• Administer medications and treatments as prescribed
• Monitor residents for changes in condition and report to RN Supervisor or physician
• Document all care provided accurately and timely in EMR system
• Communicate effectively with residents, families, and interdisciplinary team
• Maintain compliance with safety, infection control, and facility policies
• Respond to emergencies and follow established protocols
• Participate in shift handoffs, training, and care planning
