American Homecare in Los Angeles is seeking a Case Manager (LVN/LPN) to coordinate home health services and lead care across admission, treatment, and discharge. You will collaborate with families and healthcare teams to ensure timely, patient-centered care.

Qualifications include graduation from an LVN program and an active California LVN license. Strong clinical knowledge, decisive judgment, and independent work are essential. KanTime EMR experience is a plus.

Your Key Responsibilities Include

• Managing care for Home Health patients throughout the care continuum, including pre-admission, admission, and post-admission stages.
• Coordinating services to ensure that patients' needs are met in a timely and effective manner.
• Collaborating with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on patients needs.
• Monitoring the progress of patients and ensuring appropriate interventions are implemented when necessary.
• Facilitating smooth transitions of care, ensuring all services and resources are in place for each patient.
• Advocating for patients and families, ensuring their needs are met, and providing education as needed.

Must Have

• Successfully graduated from an approved and certified LVN program
• Current and unencumbered license to practice as an LVN in the state
• Ability to make quick judgments and work independently
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice To Have

• 2 years of experience as an LVN/LPN
• KanTime EMR experience
