Job summary

Community Medical Centers (CMC) is seeking a Licensed Vocational Nurse (Nursing Care Coordinator) at our CMC Lodi location to provide skilled nursing care within the team-based care model. You will collaborate with MDs, PAs, NPs, and RNs to support patient care across ambulatory settings.

The NCC reports to Nursing Leadership/CNO, ensuring facilities, tools, and support are available to deliver excellent patient care. Travel between sites may be required as needed.

Qualifications

• Current licensure as a Licensed Vocational Nurse in California.
• Proficient with Microsoft Office applications.
• Proficient using an Electronic Health Records system/s.
• Next Gen EHR experience is preferred.
• Ability to travel between sites as needed.
• Ambulatory care nursing experience preferred.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Promotes the mission, vision, and values of Community Medical Centers.
• Implements standardized nursing procedures for nurse-delivered care.
• Cares for all patients in collaboration with the healthcare team using motivational interviewing to set self-management goals.
• Communicates clearly and effectively with patients and staff.
• Oversees medication administration and storage/handling at the site.
• Delegates tasks to unlicensed staff per scope of practice.
• Provides patient education within scope of practice.
