Job summary

Alliance Health Services, Inc. is seeking a Licensed Vocational Nurse (LVN) Care Coordinator to ensure coordinated patient care and managed services within our home health program. You will review cases, assign clinicians, and guide care plans to promote quality outcomes.

Qualifications

• Licensed vocational nurse with current licensure to practice in the State.
• Nursing degree is preferred.
• Two years case management experience within the past five years in a home health agency.
• Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.
• Complies with accepted professional standards and CPR certification.
• Demonstrates excellent observation, verbal and written communication skills.

Responsibilities

• Receives case referrals and assigns clinicians to cases.
• Reviews and evaluates each case and guides clinicians to improve care delivery.
• Reviews patient diagnoses, medications, procedures and clinical course.
• Assists in establishing immediate and long-term therapeutic goals and plan of care.
• Attends case conference meetings to coordinate care.
• Performs quarterly record reviews and communicates findings to Clinical Director.
• Assists in screening and orientation of new personnel and in staff education programs.
• Assists in planning and evaluating policies, procedures, and strategic goals.
• Promotes customer service orientation and compliance with professional standards.
