Expect active collaboration with clinical staff and leadership to ensure continuity of care and compliance.

Role involves occasional patient visits for urgent coverage, with a seven-day-on-call rotation monthly. Requires CA LVN license, CPR, and 2+ years of home health case management experience.

Qualificações

• Active California LVN license.
• 2 years home health case management in last 5 years.
• CPR certification held throughout employment.
• Knowledge of care coordination and quality management.
• Strong organizational and communication skills.

Responsabilidades

• Review referrals, diagnoses, meds, and treatment plans to coordinate care.
• Assign clinicians based on patient needs and care plans.
• Collaborate with staff and leadership to address care concerns.
• Provide clinical guidance during operating hours.
• Conduct quarterly clinical record reviews for quality and compliance.
• Identify improvement opportunities and report to Clinical Director.
• Assist with staff recruitment, onboarding and education.
• Participate in case conferences and quality initiatives.
• Conduct patient visits as needed for urgent coverage.
• Complete assessments and follow-ups for assigned patients.
