WellTech Partners in Oxnard, CA is seeking a Licensed Vocational Nurse (LVN) Care Coordinator to oversee patient care coordination, clinical oversight, and quality assurance in home health. The role collaborates with clinical staff and leadership to maintain regulatory compliance and ensure high-quality services.

Primarily office-based with occasional patient visits for urgent coverage, this position includes a seven-day on-call rotation and a Monday–Friday schedule with in-person work.

Formación

• Current, active California LVN license.
• At least two years of home health case management experience in the past five years.
• Current CPR certification maintained throughout employment.
• Knowledge of patient care coordination, clinical assessment, and quality management.
• Strong organizational, time-management, communication, and problem-solving skills.

Responsabilidades

• Review patient referrals, clinical data, diagnoses, medications, and treatment plans to coordinate care.
• Assign clinicians based on patient needs and support plans of care.
• Collaborate with clinical staff and leadership to address patient care concerns.
• Provide clinical guidance and support during regular operating hours.
• Conduct quarterly clinical record reviews for quality and regulatory compliance.
• Identify improvement opportunities and communicate findings to the Clinical Director.
• Assist with staff recruitment, onboarding, orientation, and continuing education.
• Participate in case conferences and quality initiatives.
• Conduct PRN patient visits for urgent coverage and scheduling gaps.
• Complete required assessments and follow-up for assigned patient visits.
