Job summary

Qualifications

• Active California LVN license.
• At least two years of home health case management in the past five years.
• Current CPR certification.
• Knowledge of patient care coordination and quality management.
• Strong organizational and communication skills.

Responsibilities

• Coordinate care by reviewing referrals, diagnoses, meds, and treatment plans.
• Assign clinicians based on patient needs and care plans.
• Collaborate with staff to address patient care concerns and ensure continuity.
• Provide clinical guidance during operating hours.
• Conduct quarterly clinical record reviews for quality and compliance.
• Support recruitment, onboarding, and ongoing education of staff.
• Participate in case conferences and quality improvement activities.
• Complete assessments and documentation for assigned patient visits.
