Job summary

WellTech Partners in Victorville, CA is seeking an LVN Care Coordinator for home health. Pay is $35–$45 per hour, with a Monday–Friday schedule and a primarily office-based, in-person role.

The position includes one seven-consecutive-day on-call rotation each month. You will support patient care coordination, clinical oversight, and regulatory compliance, with occasional patient visits to cover urgent needs.

Qualifications

• Current, active California LVN license.
• At least two years of home health case management within the past five years.
• Current CPR certification.
• Strong organizational and communication skills.

Responsibilities

• Review patient referrals, clinical information, diagnoses, medications, and treatment plans to coordinate appropriate care.
• Assign clinicians based on patient needs and support individualized plans of care.
• Collaborate with clinical staff and leadership to address patient care concerns and promote continuity of services.
• Provide clinical guidance and support to personnel during regular operating hours.
• Conduct quarterly clinical record reviews to evaluate documentation, quality of care, and regulatory compliance.
• Identify improvement opportunities and communicate findings to the Clinical Director.
• Assist with staff recruitment, onboarding, orientation, and continuing education.
• Participate in case conferences, quality initiatives, and organizational activities.
• Conduct PRN patient visits when needed due to urgent coverage needs, staffing limitations, or scheduling gaps.
• Complete required assessments, documentation, and follow-up for assigned patient visits.
